1) Documentación requerida en admisión para hospitalizaciones programadas
- Cédula de Identidad.
- Orden de Hospitalización extendida por su médico tratante
- Consentimiento informado firmado por el paciente o apoderado responsable de su hospitalización.
- Se solicitará como documento de respaldo Pagaré Notarial con fotocopia del rut del girador del documento.
- Consulte si su empresa tiene convenio con la Clínica. De ser así, se le solicitará una Orden de Atención o Carta de Resguardo de ésta. Si incluye montos por hospitalización, se le pedirá un pagaré notarial.
- Es muy importante que traiga todos sus exámenes realizados anteriormente.
- En los casos de ingreso por diagnóstico de origen traumático, deberá completar el formulario de "Notificación de Accidente".
- En caso de que el paciente esté acogido al Seguro Catastrófico, debe presentar una carta de resguardo emitida por su Isapre.
2) En caso de maternidad
Si el Recién Nacido está inscrito en una Institución Previsional de Salud distinta a la de la madre, deberá ser indicado al Ejecutivo de Admisión.
Recuerde que el Recién Nacido debe ser inscrito en una Institución Previsional de Salud al 7º mes de su embarazo o durante los primeros 30 días del nacimiento, para que acceda a los beneficios de salud.
Se recomienda realizar un pre-ingreso desde las 37 semanas de embarazo. La madre debe traer:
Exámenes de Laboratorio.
- Ecotomografías.
- Carné de Control de Embarazo.

Una vez producido el nacimiento, debe hacer llegar a Admisión el Certificado de Nacimiento, Formulario de Incorporación a la aseguradora y Vigencia Inmediata en caso de que el nacimiento se produzca antes de la vigencia en la Isapre.
